When you come for your appointment please remember to obtain and bring the following

• Referral letter from GP or other doctor
• Medicare card
• Reports, X-rays, MRI, CT scans etc. and any other relevant information
• Your Child’s Blue Book
• Any previous  health reports (Example :  Blood test, hearing ,  speech pathology, physiotherapy or occupational therapy, etc)
• Copy of MRI/CT Scan on a disc/USB  if done externally
